Form G-325A, Biographic Information, is a supplemental form that is used with a primary application or petition.
For example, the form must be prepared by both the petitioner and beneficiary when a spouse files Form I-130 to request a green card for his or her spouse.
Make sure that the information you provide in the primary application or petition (i.e. Form I-130) is consistent with the information that you provide on Form G-325A, Biographic Information. Depending on why the form is required, USCIS will do a background check on the U.S. petitioner or the immigrant applicant.
File Form G-325A with the application or petition that requires it. Simply paper clip G-325A behind the application or petition. There is no filing fee for Form G-325A, but you will need to pay the filing fee for the primary application or petition that's being filed with USCIS.
