How should I enter name information on Form I-90?

Provide your full legal name. Your family name is the same as your last name, or surname. Spell out middle names completely and do not use just the initial. If you do not have a middle name, you may leave this field blank. Unless you have legally changed your name, your name should be exactly as it appears on your Permanent Resident Card. If you have changed your name since your existing Permanent Resident Card was issued, this is your new legal name.
